Metronome Family Medicine is a Direct Primary Care practice serving Longmont and Boulder County, founded in 2026 by Dr. Brian Juan, a board-certified family physician and University of Colorado School of Medicine graduate.

After nine years in a large health system, Dr. Juan opened Metronome to practice medicine the way he always believed it should work: with time, direct access, and real relationships. Members pay a flat monthly fee — no insurance billing, no copays — for unlimited primary care, appointments of 30 to 90 minutes, same- and next-day availability, and their doctor's direct phone number to call or text. The practice reached 100 members within its first few months.

Alongside full-scope family medicine, Metronome offers a focus on midlife vitality: hormone optimization, thyroid and metabolic health, weight management, and longevity care. An on-site pharmacy and wholesale-cost labs keep care affordable.

Metronome also partners with local employers. Direct Primary Care can be offered as a standalone benefit or paired with a leaner catastrophic plan, giving teams same-day access to a physician while often lowering an employer's total healthcare spend. Employer group rates are available.
